The Health Care Program for Children in Foster Care (HCPCFC) is an essential part of the local Child Welfare Department. Public Health Nurses (PHN) provide consultation, oversight, and management of the medical, dental, behavioral, and developmental needs of youth in out of home placement.
Services
Public Health Nurses collaborate with social workers, probation officers, medical providers, and other agencies to provide children in foster care access to appropriate health care and treatment.
Physical exam requirements:
All children in foster care are required to receive a physical exam within 30 days of placement.
Infancy

3-5 days old exam by 1 month

2 months old exam every 2 months

Early Childhood

Middle Childhood
Adolescence
Medical exams may include:
· Medical history and physical exam
· Dental assessment
· Nutrition assessment
· Developmental/behavioral assessment
· Anticipatory guidance
· Body measurements
· Hearing and vision screening
· Procedures, tests, and/or other laboratory tests as needed
Dental exam requirements:
All children in foster care are required to receive a dental exam after 12 months of age within 30 days of initial placement. Foster care children aged 1-20 years should have a dental exam every 6 months.
What to bring to your foster child’s medical and dental appointments:
- A copy of the placement agreement
- Health Contact Form for the provider to fill out
- Dental Contact Form for the provider to fill out
- Health and Education Passport
- Vaccine (shot) record
- Medi-Cal card
- Any available health records
- List of questions and concerns
After each appointment, complete a Health Contact Form. Submit the completed form to HCPCFC via email [email protected] or fax 707-784-8480.
Health and Education Passport (HEP):
A Health and Education Passport (HEP) is created for every child in foster care. It includes all known available past and current health, dental, and education information. A new or updated HEP maybe requested from the Foster Care Social Worker every 6 months to a year.
