How can I continue full scope Medi-Cal for my child after the temporary 2 month coverage is over?

 

At your child's CHDP exam, ask for a joint Medi-Cal & Healthy Families application. Fill out the application and return it to the state Medi-Cal office in the envelope provided. If you need help filling out this application, call the CHDP Program at 707-784-8670 or 1-800-826-3452. We can refer you to a certified application assistor, who will help you complete the application.

How do I use my child's temporary full scope Medi-Cal?

 

If your child receives temporary full scope Medi-Cal from the CHDP doctor's office, you will receive a paper with your child's temporary Medi-Cal number on it. You can use this paper to get medical and dental services. Within a week, you will receive a plastic Benefits Identification Card from the state Medi-Cal office. You can use this temporary card at any Medi-Cal provider office for services, such as dentistry, laboratory, pharmacy, etc. The Medi-Cal or Denti-Cal provider will verify your temporary coverage at every visit.


How often can my child get a CHDP exam?
Children can have a CHDP exam during all of these age ranges:

What do I need to bring to a CHDP doctor's visit?

 

If your child does not have full-scope Medi-Cal, the CHDP doctor's office will ask you to fill out a one-page CHDP Pre-Enrollment Application that asks for your family income. Proof of residency or income is not required.

If your child has full-scope Medi-Cal, bring your child's Benefits Identification Card (BIC Card) to the CHDP doctor's office. This is the plastic white card that you receive from the state.


What does a CHDP exam include?

  • A complete head-to-toe physical
  • Immunizations (vaccines)
  • Growth and development check
  • Teeth and gum check
  • Hearing and vision test
  • Nutrition check
  • Test for anemia, lead poison, and other lab tests
  • Health education


What if the CHDP doctor finds a medical or dental problem during a CHDP exam?

 

The cost of needed medical care will be paid by the CHDP Program through Medi-Cal. This includes follow-up appointments with other doctors, medications, eye glasses, etc.

The cost of needed dental care will be paid by the CHDP Program through Denti-Cal.


What is the CHDP Program?

 

The Child Health and Disability Prevention (CHDP) Program is a statewide preventive health program which provides free well-child health examinations and services to eligible infants, children and teens. Eligible children are able to obtain 2 months of temporary Medi-Cal coverage.


Who is eligible for a CHDP exam?

 

Low income children from birth to 19 years of age
Children with Medi-Cal from birth to 21 years of age
